We'll take the time to learn about the individual's needs, goals, and current situation. This conversation helps us understand what kind of placement and support will be the best fit. We also review Medicaid Waiver eligibility and walk you through next steps.

 

Step 3 — Personalized Care Planning

Our team collaborates with the individual, their family, and any existing care providers to develop a comprehensive, individualized support plan. This plan becomes the foundation of all the care we provide.

 

Step 4 — Matching with a Sponsor

We thoughtfully match each participant with a trained and compatible sponsor (caregiver). We consider personality, needs, lifestyle, and environment to ensure the placement feels like home — not just a placement.

 

Step 5 — Ongoing Support & Communication

After placement, our work doesn't stop. We provide continuous oversight, regular check-ins, and open communication with families. As needs evolve, so does the care plan.
