Person-Centered Support Designed Around the Individual

Life Changing Solutions serves children and adults in Virginia who have developmental or intellectual disabilities and need structured, compassionate support to live well outside of institutional settings.

We understand that every individual is unique — with their own goals, challenges, and strengths. That's why we never offer a one-size-fits-all approach. Every placement is personalized.

ELIGIBILITY

Who May Qualify for Our Program

Our services are designed for individuals who need support to live safely and successfully in their homes ior in a family-based residential setting. If you are unsure whether your loved one qualifies, we are happy to guide you through the process at no cost. 

In-Home Support Services
In-home support is typically the first option considered. This service is ideal for indiveduals who wish to remain in their own home or with their family while receiving assistance. 
Individuals may qualify if they:

  • Have a developmental or intellectual disability and are eligible for waiver services

  • Live in their own home or with family members

  • ​Require assistance with daily living activities such as bathing,dressing, meal preparation, or medication reminders

  • Benefit from support to increase independence, safety, and community involvement

  • Do no require 24-hour supervision but need consistent, scheduled support.

Sponsored Residential Services
Sponsored residential services are considered when in-home support is not sufficient to meet the individual's needs. This option provides a safe, family-style living environment with trained caregivers. 
Individuals may qualify if they:

  • Have a developmental or intellectual disability and are eligible for waiver services

  • Are unable to live independently or safely in their own home

  • Require a higher level of supervision, potentially including 24-hour support

  • Would benefit from a structured, stable home environment

  • Are willing to live with a qualified sponsor family who provides daily care and support

Medicaid Waiver Eligible

Individuals enrolled in or eligible for Virginia Medicaid Waiver services may qualify at little or no out-of-pocket cost.

Developmental or Intellectual Disability

Individuals diagnosed with an intellectual or developmental disability (IDD) who need structured daily support.

Children & Adults

We serve both juveniles and adults who need community-based alternatives to group homes or institutional care.

Behavioral Health Needs

Those with behavioral challenges who benefit from a stable, structured home environment with trained support.
